Records Request

Please note the following when making records requests:

1. While we attempt to honor any deadlines for records requests, Mathews Counseling typically requires 10-14 business days to compete any request.
2. In most cases, Mathews Counseling charges a $50 administrative fee for records requests. Intensive or sensitive requests requiring additional time will be billed at the therapist of records private pay fee.
3. For Couple or Family records requests, Mathews Counseling must have a Release of Information signed by each individual member of the couple or family.

Please note that Mathews Counseling due diligence requires obtaining an internal ROIs from clients directly and must have an internal ROI on file before releasing records.

Please note Mathews Counseling cannot provide records via fax and while any stated deadline will be honored if at all possible, Mathews Counseling generally needs 10-14 business days to fulfill records requests.
